Yan Yangong, male, deputy chief physician, emergency department of the First Affiliated Hospital of Tsinghua University and director of the Department of Critical Care Medicine. International lecturer of the World Federation of Critical Ultrasound (WINFOCUS), lecturer of the Canadian Society of Acute Intensive Ultrasound (CCUS). Deputy Director of the Cardiac Intensive Care Committee of the Chinese Society of Critical Care Medicine, Deputy Director of the Cardiovascular Intensive Care Committee of the Cross-Strait Medical and Health Exchange Association, Secretary General of the Emergency Medical Emergency Committee of the China Association for the Promotion of Medicine and Health, and Standing Committee of the Beijing Medical Association Critical Branch Member of the Intensive Medicine Branch of the Beijing Physiological Society and the Youth Committee of the Emergency Branch of the Chinese Medical Doctor Association. Chairman of the Telemedicine Committee of the Beijing Health Promotion Association and editor of the Journal of Occupation and Health. Published nearly 30 professional literatures, and participated in 3 monographs. There are 7 patents and 3 software copyrights.

"Beijing to Guangzhou, then to Delhi, India, work for 6 hours and then take the night flight to pune, non-stop. Once again, it is not easy to promote the international severe ultrasound. Especially as the project leader." This is a recent circle of friends of Yan Yangong. From "exploring in the dark" to "leading the way for the peers", as the forerunner to introduce the intensive ultrasound technology and concept into the country at the earliest, in addition to his job, Yan Yangong's time is paved in the development, research and promotion of severe ultrasound applications.

When he saw Yan Yangong, he had just finished preparing experimental animals. After the interview, he had to take the participants to participate in the acute and serious ultrasound training class of Tsinghua First Affiliated Hospital. Yan Yan’s public memory is unclear. This is the first training class held. The mobile phone is all about asking when to start the class. “The semi-annual quota is full.”

Major changes in ultrasound

In 2011, on the plane returning from France, a sudden broadcast of anxious doctors, some people fainted. Yan Yangong used to ask about the situation. The noise on the plane was very loud. The stethoscope could not be heard clearly, and the pulse was touched. After learning that the patient had a history of diabetes, Yan Yangong relied on experience to judge hypoglycemia.

The result is also as expected by Yan Yangong. The ending is also gratifying. After the sugar is added, the patient is fine. At that time, there was only one thought in Yan Yan’s public heart. If there is a portable ultrasound system, how good!

"In the 1970s, there was ultrasound. Everyone was used to waiting in line to check and wait for the results. In the last decade, with the development of miniaturization and portability, ultrasound is moving toward 'visual stethoscope'. Direction development." Yan Yangong introduced that the ultrasound system has now become the size of a notebook or even a mobile phone, and the doctor can take it to the patient at any time.

As a monitoring and evaluation tool for critically ill patients, severe ultrasound has become a hot issue in the international critical field. Due to the dynamic, real-time and reproducible characteristics of ultrasound, it can be used not only for disease assessment, but also for dynamic monitoring, obtaining important monitoring and evaluation data that cannot be obtained by other monitoring methods, and providing timely adjustment for treatment adjustment for critically ill patients. Accurate guidance.

"This is a major change, and ultrasound can play a very big role." Yan Yangong said that if a person has a car accident or a broken spleen, he would have to use a flat car to pull the patient to the ultrasound department to check. Not only does it take a lot of time, but problems can occur during the patient's transfer process. Now, with the use of portable, severe ultrasound technology in the emergency department or even the ambulance, the patient has a quick and accurate understanding, and the doctor is ready. The rescue has greatly improved the efficiency of treatment. Because of this change, more and more doctors come to the patient with small instruments.

From their own exploration to the core members

Yan Yangong began to contact with severe ultrasound in 2007. "At that time, no one in China did this. At the time, there were some documents in foreign countries, because there were not many people doing such research and application in the world."

Pulmonary ultrasound can be used as an important tool to help doctors identify and diagnose many clinical emergencies? After seeing reports from foreign countries, Yan Yangong felt very magical. However, there was no one in the country who was engaged in this work. Therefore, Yan Yangong himself studied foreign literature.

Without the guidance of the master, Yan Yangong tried to explore the research.

"Ultrasound is to see the image, there is no image, just listen to what people say is A line, what is B line, it is a whole paste." So as long as there is a picture on the foreign website, he will survive.

He also specialized in finding software for engineers to record the ultrasound data at the bedside inspection and on the Internet, and then compare the literature to gradually understand how it is applied. After a period of research, Yan Yangong slowly had some experience and experience.

In more than two years, Yan Yangong has accumulated a lot of problems. Without his peers, he is eager to find someone to point out. In 2009, WINFOCUS (World Severe Ultrasound Alliance) held an academic event in Thailand with a training session. After learning about it, Yan Yangong rushed to Bangkok at his own expense and lived in a small restaurant next to a hospital. He took classes during the day and completed a draft in the evening.

WINFOCUS experts have noticed that there is only one Chinese among the more than one hundred participants. Moreover, this Chinese is still very active, not only actively asking questions in class, but also chasing experts after class, and taking out the problems and cases accumulated by them and discussing with experts.

The conference brought together a number of top experts including the founders of WINFOCUS. They found that this young man from China was not only learning. He not only understood the concept of severe ultrasound, but also did a lot in China. work. Just as WINFOCUS wants to promote the application of clinical ultrasound worldwide, it is said to Yan Yangong that since you have done so much work, would you like to join this team and promote it to the whole world?

One year later, Yan Yangong went to Europe as an international training lecturer to participate in a full-scale training for several months, and comprehensively studied the application of severe ultrasound and training and communication skills in different centers in Italy and France. In this process, Yan Yangong also met many European and American experts. Everyone exchanged a lot and discussed a lot. Foreign experts also found that Yan Yangong had a lot of work in front of them in the research and development and application of medical professional software, so he asked if you would like to become a core member of this organization.

Therefore, Yan Yangong became a member of the global core team of WINFOCUS, responsible for the support of information technology. After 2012, Yan Yangong began to participate in the forum on medical technology progress organized by the United Nations, and communicated and delivered a special speech.

Let domestic severe ultrasound development and international synchronization

Recently, the 4th Annual Taihu Lake Trauma Emergency Forum and the “Shanghai Emergency ICU Quality Control Center Bedside Ultrasound Technology Training Class” were held. Yan Yangong specially lectured “Emergency Shock: Focused Processed Ultrasound Management”, which was brought to you. The latest advances in ultrasound and international concepts have given you a new understanding of the application and understanding of ultrasound.

But Yan Yangong remembers that when he first gave a public speech in China, he was not as lively as it is now.

In 2011, Yan Yangong brought the severe ultrasound back to China and promoted lectures on different occasions. One lecture was held at the national critical annual meeting. "There are only 15 people attending the 400-person venue." But what Yan Yangong saw hope was that the students who were present at the time were all domestic experts.

With the rapid development of international severe ultrasound and the accumulation of evidence-based medical evidence, domestic clinical ultrasound has gradually become a hot spot. Under the guidance of the famous expert Professor Liu Dawei, Beijing Union Medical College Hospital Wang Xiaoting and Yan Yan Public jointly promoted the establishment of the China Severe Ultrasound Research Group, and began to work on the training, promotion and future high-level scientific research cooperation of Severe Ultrasound in China.

Training is the foundation for development and promotion, and quality control is the driving force for sustainable development. Because of the strong practical operation requirements of ultrasound technology learning, Yan Yangong has innovatively established a model of pneumothorax, pleural effusion and ARDS animal pathology to facilitate the actual operation of the students. In order to ensure the quality of learning, each training is a small class, so the training class is very hot. "It is usually half a year ago to register." A few years later, emergency, severe, anesthesia, and even cardiology and respiratory doctors. Within, a large number of clinical ultrasound doctors have been trained to effectively promote the development of severe ultrasound in China.

However, their work did not stop there.

With the rapid development of miniaturization, ultrasound as a visual diagnostic and monitoring tool combined with information technology is breaking through the limitations of traditional medical time and space, greatly improving the accuracy and accessibility of clinical diagnosis and treatment and reducing medical expenses. Therefore, Yan Yangong cooperated with WINFOCUS to combine ultrasound with pre-hospital and in-hospital emergency medical treatment to establish a remote mobile visualization emergency treatment system, which largely solved the adverse effects of time and space on the treatment of patients with acute and severe diseases. Improve the level of treatment for critically ill patients. Due to the innovative advantages of the information technology platform, the system has been used by the Italian government for an important support platform for the acute and serious treatment of the Milan Expo this year. In China, it has also carried out networked ICU and four-in-one demonstration projects in the departments of critical medicine, emergency department, pre-hospital first aid and social health management in Shenzhen, Jiangsu, Shanxi and Beijing.

"Visual medical support under the Internet +, the prospects are great!" Yan Yangong said.
